It Starts with a Referral on Behalf of a Child
How to Get Help
If your child has been diagnosed with a critical, non-cancerous medical condition, Hearts Aligned may be able to provide financial assistance, family support, and advocacy services. We understand that caring for a critically ill child can be overwhelming, and we’re here to help guide you through the referral process.
This is not an application but will allow our team to contact you and determine the next steps.
Eligibility
To qualify for Hearts Aligned’s programs, families must meet the following requirements:
– Have a child under the age of 18 with a critical, non-cancerous medical condition or a non-cancerous terminal diagnosis.
– Reside in Santa Barbara County.
– Meet the U.S. Department of Housing and Urban Development (HUD) Very Low Income guidelines for Santa Barbara County.
– Have a child who meets our medical eligibility criteria, including a recent hospitalization of 4 or more nights, a terminal diagnosis, or other qualifying medical conditions as determined by Hearts Aligned.
Referral Process
Hearts Aligned does not have an online application.
If you believe your family may qualify, please speak with your hospital social worker or healthcare provider. They can submit a referral and any supporting medical documentation on your behalf.
Referrals can be emailed to referrals@heartsaligned.org
