Case Manager – North Santa Barbara County
The Case Manager will be responsible for our family support programs in North Santa Barbara County. The Case Manager will work with the Executive Director to ensure Hearts Aligned programs are being administered properly. The Case Manager will work with medical referral sources and ensure all families have completed all required forms; are assessed for eligibility; interviewed; and have a well-documented file to be accepted into one or more of Hearts Aligned programs.
We are pleased to share some of the duties and responsibilities associated with the position.
Financial Assistance
- Work with each healthcare professional referral source to obtain a recommendation for service
- Review all applications to ensure they are complete
- Contact parents/guardians and conduct initial phone interview and review of the application
- Conduct in-person interviews with the family
- Keep Family Information file and database up to date
- Document all interactions with the family
- Make a report of the recommendations to approve or reject application to the Executive Director
- Send approval and/or rejection letters to every family that has applied
- Interact with Social Workers and Case workers/managers, as necessary
- Facilitate family grants – collect bills/invoices, W-9 forms and complete expense forms. Invoices will be processed by another individual.
- Follow-up with families to check on their welfare on a regular and ongoing basis
Personal Support
- Work with the Project Manager for program item deliveries
- Determine type of volunteer needed and schedule volunteers
- Provide assistance to those families in need of transportation (referrals), household needs, shopping, etc.
Patient Advocacy
- Coordinate other community resources for families – Foodbank, CalFresh, CenCal Health, etc.
- Continually update available resources as new ones are discovered
- Assist families with understanding health insurance bills and negotiating payments for past due or current bills
Fundraising/Community Engagement
- Participate in fundraising activities
- Attend events to promote community awareness
- Promote Hearts Aligned as appropriate
- Collect family stories for publication
POSITION REQUIREMENTS:
Candidate should have excellent written and verbal communication skills in English and Spanish.
Bachelor’s Degree preferred. Case management experience of 3-5 years preferred.
Computer skills are required, including the use of Google Drive and databases. The candidate should
have excellent organizational and time management skills. Experience working with families in need is
preferred. Must have reliable transportation. Must have reliable internet access/connection. Successful
background check is required.
WORK ENVIRONMENT
The employee must live in North Santa Barbara County and will begin working remotely until office is set up. Employee must have reliable internet access. Equipment supplied by Hearts Aligned is to be used for business purposes only.
COMPENSATION
$25-$30 per hour depending on experience, full-time in Santa Maria
Benefits include accrued vacation (after 90 days employment), holiday pay, sick leave
